Important Information Regarding Covered Services

In compliance with Florida law, we are obliged to inform our patients and potential patients about the following:

At Ospine Surgical Center, healthcare services may be administered by the facility itself or by other healthcare providers. These providers may bill patients separately, and their participation with health insurers or organizations might differ from that of the facility.

Patients may or may not find more affordable services in alternative facilities or healthcare settings. Additionally, the attending physician who schedules the patient’s procedure at our facility might not be affiliated with other similar establishments.

Please note that Ospine Surgical Center does not employ its own physicians. Each physician or service provider will bill patients separately, following their individual billing and collection procedures. Except for the facility itself, no other providers offering medically necessary services at the facility are covered under this policy.

Request for Charge Estimates:

As per Florida law, upon request, Ospine Surgical Center will provide patients with a written or electronic estimate of the facility’s expected charges for their procedure(s) within seven (7) business days. This estimate is based on the physician/surgeon’s recommendation for treating the patient’s condition. If the patient is insured, contracted insurance rates will override the gross charges estimate.

During any medical procedure, unforeseen circumstances might require additional or different procedures or the use of more/less expensive supplies or implants. Such variations could significantly affect the provided estimate. The final charges and patient responsibility depend on the actual services rendered and may exceed the original estimate.

The charge estimate provided pertains to the facility only. Patients seeking estimates from other healthcare providers within the facility should contact each provider and inquire about their network participation with the patient’s health maintenance organization (insurance company).

Ospine Surgical Center’s policy mandates the collection of deductibles, co-insurances, and copayments, as determined by the patient’s insurance company, before or on the day of surgery. If the patient lacks insurance, Ospine Surgical Center will collect 100% of the gross charges.

Patients can contact their health insurer or health maintenance organization (insurance company) for detailed information about cost-sharing responsibilities.

Financial Assistance:

Patients facing difficulties paying their estimated deductible, co-insurance, or copayment can opt for Care Credit, which allows for manageable monthly payments. Detailed information about the Care Credit program, including the application process, will be provided to the patient.

Charity Care:

Patients unable to cover their procedure’s cost share due to medical indigency or lack of insurance may request a discount or a payment plan from the Facility. Such discounts are considered charity care, and there is no formal application process for obtaining it at the Facility.

It’s important to note that the attending physician and other healthcare providers, such as anesthesiologists, durable medical suppliers, pathologists, or laboratory service providers, are not included in the Facility’s fees. These providers will bill patients separately for the services they render and may have different affiliations with health insurers or health maintenance organizations compared to the facility.

Exceptions to this Policy:

The Facility’s Business Office Director, Chief Administrative Officer, or Chief Executive Officer has the authority to consider eligibility and make exceptions to this Policy on a case-by-case basis, taking into account individual patient circumstances.
